Trump administration begins sending $500 Obamacare refund checks to nearly 1 million Americans

Obamacare refund checks worth $500 are being sent to more than 950,000 Americans across 30 states. The payments target certain HealthCare.gov customers who paid full-price premiums without federal assistance, Baltimore Chronicle reports. The program was announced earlier by the White House, while federal marketplace information is available through CMS.

Who will receive the $500 Obamacare refund checks

Treasury checks are expected to reach people enrolled through the federal Affordable Care Act marketplace. Administration officials say more than 950,000 people qualify.

Most recipients paid their insurance premiums without receiving premium assistance. Many have incomes above 400% of the federal poverty level.

The payments generally apply to:

  • eligible HealthCare.gov customers in 30 states;
  • people who paid the full cost of ACA coverage;
  • some consumers earning between 100% and 400% of the federal poverty level who received no subsidy.

People receiving premium subsidies are generally excluded. Residents of states operating their own ACA exchanges are also outside the federal refund program.

The White House initially said checks would begin going out in October 2026. Reports published September 30 said the Treasury mailing process had already started.

Texas and Florida have the largest number of recipients

The payment is $500 per eligible person rather than per household. That means some families could receive more than one payment when several members qualify.

Why the administration is issuing the refunds

The White House says the money comes from roughly $500 million in surplus HealthCare.gov user fees. Insurance companies pay these fees to support operation of the federal marketplace.

The administration argues that the fees were ultimately reflected in consumer premiums and produced more revenue than required to operate the exchange. That characterization is the administration’s position rather than an independently established finding.

President Donald Trump told recipients that the government was returning money that, in his words, had been collected excessively under the previous administration.

User fees have traditionally helped finance services including technology, call centers and enrollment activities.

Refunds arrive as ACA costs remain a political issue

The payments come shortly before the November 2026 midterm elections, when healthcare affordability is a major campaign issue.

Enhanced ACA premium subsidies expired at the end of 2025. Consumers who lost that assistance have faced substantially higher premiums in 2026.

The $500 Obamacare refunds are separate from Trump’s proposed $5,000 payments. That proposal has been tied publicly to Republican control of Congress and is not part of the current ACA refund program.
