A 15th victim has died from injuries sustained in the UPS plane crash at Louisville Muhammad Ali International Airport on November 4, 2025, according to Baltimore Chronicle with reference to statements from Kentucky Governor Andy Beshear. Governor Beshear announced on social media that Alain Rodriguez Colina succumbed to his injuries, bringing the total fatalities from the crash to fifteen.
The McDonnell Douglas MD-11 aircraft was departing Louisville for Daniel K. Inouye International Airport in Honolulu when the crash occurred. According to the National Transportation Safety Board (NTSB), the aircraft’s left engine and left pylon separated from the wing during takeoff, igniting a fire that spread over the fuselage before the plane struck the ground. Additional fire was reported near the left pylon attachment to the wing, persisting until ground impact, according to the NTSB’s preliminary report.
The plane only reached an altitude of approximately 30 feet before crashing in a ball of fire, impacting a storage yard and two buildings. Post-crash inspection revealed that the left engine pylon exhibited fatigue cracks and signs of over-stress failure, the NTSB noted.
The crash resulted in the deaths of the three crew members on board and eleven people on the ground. Additionally, 23 individuals on the ground sustained injuries. Following the incident, the Federal Aviation Administration temporarily grounded all MD-11 aircraft until inspections were completed. Approximately 70 MD-11 planes are currently in service with UPS, FedEx, and Western Global, according to Boeing, the manufacturer of the aircraft.
